Currently, almost one out of every two Americans dies of cardiovascular disease. In 1999, for a US population of about 270 million, there were 728,743 coronary artery disease deaths and 63% of them were attributed to sudden cardiac death. This makes sudden cardiac death by far the most common single cause of death in the US and probably in many other industrialized countries.
Sudden cardiac death is usually caused by sudden ventricular arrhythmias. Unfortunately, antiarrhythmic drugs have proved of limited efficacy to prevent sudden cardiac death. Contrarily, randomized trials of implantable defibrillators summarized at the end of this book demonstrate a remarkable efficacy in protecting against arrhythmic death. Protection extends to patients with compromised ventricular function but without a history of life- threatening arrhythmias. Primary or prophylactic device therapy improves survival on top of that achieved by optimum heart failure treatment including beta blockade and angiotensin modulation.
The rapid popularization of defibrillators and their increasing complexity has created a new dimension in cardiac care.
